Unusual loss of power then blue/white smoke - HELP!

Hi all, I have spent months now trying to fault find my problem by searching and reading forums, especially grateful to every one here and their posts. I am hoping for some new direction as i am now stumped. As previously stated on this forum holden dealerships are now no longer any help and diesel specialists are few and far between.

Our problem started two months ago when my wife was driving and the Jackaroo just died with a loss of power and then would not start. I managed to get the car home after continuously cranking it and finally got it started. Now it will turn over no problem normally first crank and idle fine but intermittently will lose power at around 3,000-3,500 rpms almost like a lack of fuel and slowly reduce to a rough idle and blow a heap of blue/white smoke and smell like burnt oil. Sometimes it will die completely and stop and the next start takes a little cranking to get started, almost like a lack of fuel, and then once started will idle normally until I increase the revs again. Sometimes it will run at 3,000-3,500 revs no problems for 10-15 minutes and then just die, sometimes it will do it straight away. I always run Penrite HPR Diesel 5 oil and do oil changes and both filters every 5,000kms, the car still looks new. I topped oil up to eliminate broken oil pick up and have also no fuel or oil in coolant. I have bypassed the fuel filter with an inline filter to eliminate any chance of air ingress or blockage to the fuel line, and have also blown back fuel lines to tank and drained tank, all clear and clean. Recently we hooked up a Tech 2 clone from a mechanic and revealed no Fault codes at all but one thing we noticed just as the loss of power occured again, and then rough idle, was the voltage on the ITP (Intake throttle Position) sensor spiked at 3 volts from 0.5 volts. Straight after the power died and blew smoke. I thought this may have meant under fueling as the ECM thought the throttle had closed and eased off the fuel when it actual fact it was still wide open throttle. This is not conclusive to me and I want to hook it up again to a proper Tech 2 but can't find anyone that will hire me one for a day. Just another thing to note i have also uncoupled the ITP and Throttle Motor at the Throttle body and no MIL (Malfunction Indicator Lamp) came on and ran just like normal and still did the loss of power and blow smoke. Any ideas PLEASE???????

A bit of history;
1. Has had injector seal failure at 30,000kms from new and motor blew up - Motor replaced by holden after much arguing and threatening of legal acction. 120,000kms now on the clock and has had recall on new motor for injector seals, injector wiring harness.
2. Has had Holden's recall for the acceleration issue with their crappy slicing ECM boots to splice in new loom.
3. Replace ORPS and Injector harness as well as tapper cover gasket three weeks ago, no change.
